VHCC provides NDIS Support Coordination and Specialist Support Coordination to help participants understand their NDIS plans, connect with the right services and bring their supports together around their individual needs and goals.

Level 2 Support Coordination — Box Hill
Helping participants understand and implement their NDIS plans, coordinate services and build greater confidence managing their supports.
Level 3 Specialist Support Coordination — Ballarat
Higher-level coordination for participants with complex support needs, barriers or risks that require a more specialised approach.

Finding the right supports can sometimes feel complicated. Our Support Coordinators work alongside participants to make the process clearer, bring services together and help ensure supports are working towards the participant’s goal
For participants with more complex circumstances, our Specialist Support Coordination service provides a higher level of expertise to help identify and address barriers, coordinate complex support environments and develop appropriate service approaches.
Level 3 Support for More Complex NDIS Needs
Some participants experience circumstances that make accessing, establishing or maintaining appropriate supports significantly more complex.
Level 3 Specialist Support Coordination provides a higher level of support coordination for participants with complex needs who require specialist assistance.
Jane provides Specialist Support Coordination for participants whose circumstances require a higher level of coordination and specialist support.
Working collaboratively with participants, families, providers and other professionals, Jane focuses on understanding the participant’s overall support environment and identifying the barriers that may be preventing them from accessing or maintaining appropriate services.
Her role is to help bring complex supports together, establish clear service approaches and support continuity when challenges arise.
Areas of Specialist Support
Chloe is a Senior Support Coordinator with more than five years of experience working within the NDIS sector.
She takes a strengths-based, person-centred approach and is committed to providing respectful, compassionate and non-judgemental support. Chloe works collaboratively with participants, families, providers and other professionals to help participants understand their NDIS plans and establish supports that reflect their individual circumstances and goals.
Her experience includes supporting people with psychosocial and physical disabilities, complex family circumstances and Supported Independent Living arrangements.
Chloe can provide Support Coordination to participants face to face from our Box Hill location or remotely, providing greater flexibility for participants who may not live locally or prefer remote support.
