Social & Community Participation – Purposeful Support, Focused on You.
At VHCC, Social and Community Participation is more than attending activities — it is a structured support designed to build capacity, increase independence, and promote meaningful engagement in everyday life, while supporting participants to achieve their NDIS goals.
We deliver person-centred supports that align with participant goals, focusing on developing the skills required to access and participate in the community safely, confidently, and independently over time, with an emphasis on meaningful engagement and inclusion.
Our support staff work alongside participants to identify their individual goals, strengths, and support needs, ensuring all supports are purposeful, outcome-focused, and aligned with their NDIS plan.
These supports are currently offered across:
- New South Wales (NSW): Hunter Region, Newcastle, Lake Macquarie and surrounding areas
- Victoria (VIC): Box Hill, Ballarat, Shepparton and surrounding areas
What We Support
Through Social and Community Participation supports, we assist participants to:
- Build capacity to engage in social and community activities
- Develop and maintain interpersonal and communication skills
- Increase confidence in accessing community environments
- Strengthen emotional regulation and adaptive responses in social settings
- Develop skills to participate in daily living activities within the community
- Establish and maintain meaningful relationships and social connections
- Explore individual interests, hobbies, and community-based opportunities
- Build confidence in community access, including transport and public settings
- Increase independence in accessing and navigating community environments
Our Approach
Our approach is grounded in person-centred practice, with a strong focus on capacity building and participant choice and control.
We work collaboratively with participants, their families, and support networks to deliver supports that are:
- Focused on skill development and capacity building aligned with participant goals
- Delivered using active support practices to promote participation and choice
- Strengths-based, building on existing skills and abilities
- Structured and consistent, supporting skill development over time
- Responsive to each participant’s functional capacity and support needs
- Delivered with dignity, respect, and a strong commitment to participant rights
- Guided by participant choice, control, and individual preferences
All supports are delivered in a way that promotes meaningful engagement, inclusion, and increased independence over time.
Structured & Outcome-Focused Supports
At VHCC, Social and Community Participation supports are delivered with clear intent and aligned with participant goals and funded supports under the NDIS.
Supports may include:
- Assistance with accessing community, social, and recreational activities
- Skill development in real-life environments (e.g. shopping, appointments, social settings)
- Support to build independence in daily living and community participation
- Travel training and support with transport to access the community
- Support to engage in group-based or individual community activities
- Assistance to explore education, volunteering, and employment pathways
Who We Support
VHCC provides Social and Community Participation supports to participants with:
- Psychosocial disability
- Intellectual disability
- Physical disability
- Complex support needs
All supports are tailored to each participant’s individual goals, functional capacity, and NDIS plan.
Funding Information
Social and Community Participation supports may be funded under the National Disability Insurance Scheme (NDIS) within:
- Core Supports – Assistance with Social and Community Participation
- Capacity Building Supports – Increased Social and Community Participation
The specific funding category will depend on the participant’s individual NDIS plan and goals.
VHCC works closely with participants, families, and Support Coordinators to ensure supports are delivered in line with available funding, while maintaining a strong focus on capacity building, meaningful engagement, and long-term independence.
