NDIS Funded · NSW & Victoria
At VHCC, Social and Community Participation is more than attending activities — it is a structured support designed to build capacity, increase independence, and promote meaningful engagement in everyday life, while supporting participants to achieve their NDIS goals.
Our support staff work alongside participants to identify their individual goals, strengths, and support needs, ensuring all supports are purposeful, outcome-focused, and aligned with their NDIS plan.

Our Approach
Our approach is grounded in person-centred practice, with a strong focus on capacity building and participant choice and control. We work collaboratively with participants, their families, and support networks to deliver supports that are meaningful, structured, and outcome-focused.
Focused on building skills that support participants to engage in their community safely, confidently, and independently over time — aligned with participant NDIS goals.
Delivered using active support practices that promote participation and choice, ensuring participants are actively engaged and supported to do as much as possible for themselves.
Builds on each participant’s existing skills and abilities, celebrating progress and empowering individuals to reach their individual goals and full potential.
Support routines are structured and consistent, providing the stability and predictability that helps participants develop skills and confidence over time.
Responsive to each participant’s individual functional capacity and support needs, ensuring supports are calibrated appropriately to promote independence.
Every support is delivered with a strong commitment to participant rights, dignity, and respect — ensuring participants feel valued, included, and empowered.
Supports are guided by participant preferences, interests, and goals — ensuring each individual has genuine choice and control over their support and participation.
