All nursing care is delivered by registered nurses and enrolled nurses with the appropriate clinical qualifications, AHPRA registration, and experience in disability and community health settings.

What We Offer
Our registered nurses deliver a comprehensive range of in-home clinical nursing services, aligned with your NDIS plan and health needs.
Assessment, treatment, and ongoing management of acute and chronic wounds including surgical wounds, pressure injuries, leg ulcers, and diabetic foot wounds — delivered by registered nurses in the home.
Medication administration, Webster pack management, medication reconciliation, and education for participants and carers on safe medication use and storage under registered nurse supervision.
Ongoing nursing support for participants living with chronic conditions including diabetes, COPD, heart failure, and other complex health conditions — monitoring, education, and care coordination.
Comprehensive nursing assessments including health status reviews, medication reviews, falls risk assessments, pressure injury risk assessments, and clinical care planning delivered in the home.
Urinary catheter insertion, management, and removal; continence assessments and management plans; stoma care and irrigation — all delivered by appropriately qualified nursing staff.
Nursing support for participants transitioning from hospital to home, ensuring continuity of clinical care, medication management, wound care, and safe discharge planning in collaboration with the treating team.

Our Approach
Our approach to community nursing care is built on clinical excellence, compassion, and a genuine commitment to supporting each participant to live safely and healthily in their own home.
All community nursing care is led by AHPRA-registered nurses with the qualifications, clinical experience, and competencies required to deliver safe, effective nursing care in community settings.
We develop individualised care plans in collaboration with participants, families, GPs, and allied health teams — ensuring care is tailored to each person’s specific clinical needs and goals.
All nursing care is delivered in the comfort of the participant’s own home, avoiding unnecessary hospital or clinic visits and supporting participants to remain independent in their familiar environment.
Our nurses work collaboratively with GPs, specialists, allied health professionals, and support coordinators to ensure a coordinated, whole-of-person approach to health and wellbeing.
All nursing care is delivered in accordance with current clinical evidence, AHPRA standards, and relevant professional guidelines — ensuring safe, effective, and consistent clinical outcomes.
Every nursing interaction upholds participant dignity, privacy, and confidentiality — consistent with VHCC’s commitment to person-centred care and the NDIS Practice Standards.
Who We Support
VHCC provides community nursing care to participants across a range of health and disability presentations. All nursing care is delivered by qualified, experienced registered nurses.
In-home nursing care for participants with complex physical and medical conditions requiring regular clinical monitoring, wound management, medication management, and health assessments.
Ongoing nursing support for participants living with chronic conditions including diabetes, cardiovascular disease, respiratory conditions, and other long-term health conditions requiring regular nursing review.
Nursing care for participants transitioning home from hospital, including wound care, medication management, post-surgical monitoring, and coordination with the treating medical team.
Compassionate in-home nursing for participants with age-related conditions and complex care needs, supporting them to remain safely in their own home with appropriate clinical oversight.
