At VHCC, Home & Living supports are more than accommodation — they are a structured and person-centred approach to supporting participants to live safely, independently, and with dignity in a place they can call home.

We deliver supports that are designed to build capacity, promote independence, and support participants to develop the skills required for everyday living, while ensuring their health, safety, and wellbeing are maintained.

Our supports are aligned with each participant’s NDIS plan and goals, with a strong focus on creating stable, consistent, and supportive home environments that enable long-term outcomes.

Our support staff work alongside participants to understand their individual needs, preferences, and routines, ensuring supports are purposeful, structured, and responsive.

These supports are currently offered across:

  • New South Wales (NSW): Hunter Region, Newcastle, Lake Macquarie and surrounding areas
  • Victoria (VIC): Box Hill, Ballarat, Shepparton and surrounding areas

Home & Living Support Types

VHCC provides a range of Home & Living support models to meet the diverse needs of participants, including:

  • Supported Independent Living (SIL)
    Ongoing support within a shared or individual living environment, designed to assist participants with daily living tasks while building independence and capacity over time.
  • Individualised Living Options (ILO)
    Flexible and tailored living arrangements that support participants to live in a way that suits their preferences, relationships, and goals, with a focus on informal and formal supports working together.
  • Medium Term Accommodation (MTA)
    Temporary accommodation supports for participants who are transitioning to long-term housing solutions, ensuring continuity of care and stability during interim periods.
  • Short Term Accommodation (STA)
    Short-term stays that provide participants with opportunities for respite, skill development, and community engagement in a supported environment.

What We Support

Through Home & Living supports, VHCC assists participants to:

  • Develop skills for daily living, including personal care, meal preparation, and household tasks
  • Build independence in managing routines and responsibilities within the home
  • Strengthen emotional regulation and wellbeing in a stable living environment
  • Develop communication and interpersonal skills within shared living arrangements
  • Increase capacity to make decisions and exercise choice and control
  • Maintain a safe, clean, and functional living environment
  • Build skills to sustain long-term tenancies and housing stability
  • Engage with their community while maintaining a structured home base

Our Approach

Our approach to Home & Living is grounded in person-centred practice and aligned with the NDIS Practice Standards, with a strong focus on capacity building and participant choice and control.

We work collaboratively with participants, families, guardians, and support networks to deliver supports that are:

  • Focused on skill development and increasing independence in the home
  • Delivered using active support practices to promote participation and autonomy
  • Structured and consistent, supporting routine, stability, and predictability
  • Strengths-based, building on each participant’s existing skills and capacity
  • Responsive to complex support needs, including psychosocial and behavioural presentations
  • Delivered with dignity, respect, and a strong commitment to participant rights

Our supports are designed to reduce reliance on supports over time, while ensuring participants feel safe, supported, and empowered within their home environment.

Structured & Outcome-Focused Supports

At VHCC, Home & Living supports are delivered with clear intent and aligned with participant goals and funded supports under the NDIS.

Supports may include:

  • Assistance with Activities of Daily Living (ADLs), including personal care and hygiene
  • Support with meal planning, preparation, and nutrition
  • Assistance with household tasks such as cleaning, laundry, and organisation
  • Support to establish and maintain daily routines
  • Overnight and/or 24/7 supports where required
  • Support with medication prompts and health-related routines (within scope of practice)
  • Assistance to develop independent living skills
  • Support to maintain tenancy obligations and property care
  • Implementation of behaviour support strategies in line with approved plans

Who We Support

VHCC provides Home & Living supports to participants with:

  • Psychosocial disability
  • Intellectual disability
  • Physical disability
  • Complex support needs, including high-intensity supports

All supports are tailored to each participant’s individual goals, functional capacity, and NDIS plan.

Funding Information

Home & Living supports may be funded under the National Disability Insurance Scheme (NDIS) within:

  • Core Supports – Assistance with Daily Life
  • Capacity Building Supports – Improved Daily Living
  • Home & Living supports (including Supported Independent Living – SIL)

The specific funding category and support model will depend on the participant’s individual NDIS plan, goals, and assessed support needs.

VHCC works closely with participants, families, and Support Coordinators to ensure supports are delivered in line with available funding, while maintaining a strong focus on capacity building, stability, and long-term independence.
