VHCC provides a range of Home & Living support models to meet the diverse needs of participants, designed around individual goals, preferences, and NDIS plans.
Ongoing support within a shared or individual living environment, designed to assist participants with daily living tasks while building independence and capacity over time.
Flexible and tailored living arrangements that support participants to live in a way that suits their preferences, relationships, and goals, with informal and formal supports working together.
Temporary accommodation supports for participants who are transitioning to long-term housing solutions, ensuring continuity of care and stability during interim periods.
Short-term stays providing participants with opportunities for respite, skill development, and community engagement in a supported environment.

Who We Support

We support participants living with mental health conditions and psychosocial disability, delivering structured, consistent supports that promote stability and wellbeing.
Our teams are experienced in supporting participants with intellectual disability across a range of living arrangements, with a focus on skill building and community inclusion.
We provide home and living supports for participants with physical disability, including assistance with personal care, mobility, and activities of daily living.
VHCC is experienced in supporting participants with complex and high-intensity support needs, including those requiring behaviour support plan implementation.
