Our Commitment
VHCC is dedicated to providing care that empowers individuals to live safely, independently, and confidently in their own home. Whether you need daily support, weekly check-ins, or complex clinical care, our team is here to help.
Who We Support
VHCC provides in-home care to participants across a wide range of disability types, medical needs, and life stages — all delivered safely and with dignity in the participant’s own home.
In-home care for participants with complex physical conditions including wound care, PEG feeding, continence management, mobility assistance, and medication administration.
Person-centred in-home care for participants living with mental health conditions — structured, consistent, and delivered with sensitivity to the unique challenges of psychosocial disability.
Compassionate in-home care supporting older participants to remain safely and independently in their own home, including nursing care, domestic help, and social support.
Specialist in-home support for participants living with ABI, stroke, MS, or other neurological conditions — including behaviour support, clinical care, and daily living assistance.
Our Approach
Our approach combines clinical excellence with genuine compassion — ensuring every participant receives care that is safe, skilled, and deeply person-centred.
Every care plan, every interaction, and every decision is guided by what matters most to the participant — their goals, preferences, values, and lifestyle.
Support Workers and Registered Nurses are appropriately trained, screened, and experienced — ensuring every participant receives safe, competent in-home care.
We match participants with the same support workers wherever possible, building trusted, familiar relationships that make in-home care more effective and comfortable.
From a few hours a week to full-time 24/7 care — we build schedules around the participant’s routines, preferences, and NDIS funding.
We work closely with GPs, allied health professionals, Support Coordinators, and families to ensure every participant receives coordinated, whole-of-person support.
Our multicultural team includes support workers and nurses from diverse linguistic and cultural backgrounds — ensuring care is always respectful and inclusive.
