- Case Management
Our highly skilled case management team is integral to the success of the services we offer. Case managers work directly with participants and their families to develop and oversee personalized care plans. They ensure that each participant receives the right level of support tailored to their specific needs. Case managers coordinate all aspects of care, including arranging and supervising support workers, facilitating communication between healthcare providers, and adjusting plans as needed. This ensures that participants receive consistent and effective care that evolves with their needs. The Case Managers work closely with external and internal Support Coordinators to ensure the participants are supported with high-quality supports.
- Transition from Hospitals
One of the most significant challenges for individuals leaving hospital care is transitioning to a more permanent, independent living situation. VHCC offers vital transition services that support individuals as they move from hospitals or other institutional settings to homes where they can live more independently. This service is essential for individuals who may still require significant assistance with physical or mental health management but are ready to begin living in a community-based setting. Our team provides hands-on support to ensure participants receive all necessary health care, assistance with daily living activities, and a safe and supportive environment as they adjust to their new living arrangements.
- After-Hours On-Call Service
We recognize that care needs don’t always fit into regular business hours. VHCC’s after-hours on-call service provides an added layer of support for participants and their families, offering access to professional assistance when needed most. Whether it’s a medical emergency, a change in the participant’s condition, or unexpected support requirements, our trained professionals are available to offer immediate help and guidance, ensuring peace of mind for participants and their loved ones at all times.
- Care Workers
Our team of over 150 trained and dedicated care workers are the backbone of VHCC’s services. Each care worker undergoes comprehensive training to ensure they are equipped with the skills and knowledge to handle the unique needs of individuals with complex physical and mental health challenges. Care workers are compassionate, respectful, and skilled in providing hands-on support to help with activities of daily living, mobility assistance, personal care, and more. Through regular training and supervision, we ensure that our care workers provide the highest standard of care in a respectful, empathetic, and professional manner.
- Housing Options
We offer a wide variety of housing options that cater to different needs and preferences, ensuring participants can choose the best fit for their lifestyle and level of independence. Our housing options include:- Supported Independent Living (SIL): Designed for individuals who require assistance with daily living tasks but wish to live as independently as possible. SIL offers a blend of personal care and support while promoting autonomy in a home-like environment. Care workers assist with daily activities, medication management, and personal care needs, providing the right level of support to foster independence.
- Independent Living Options (ILO): Tailored for individuals who are capable of managing most aspects of their daily life independently but may need minimal assistance. ILO programs are flexible and designed to promote freedom and self-sufficiency while ensuring participants have access to support when necessary.
- Short-Term Accommodation (STA): Ideal for individuals who need temporary housing solutions while transitioning between living arrangements or requiring short-term respite care. STA offers a safe, comfortable environment with the necessary support to manage short-term needs.
- Medium-Term Accommodation (MTA): For individuals who require temporary housing while they await long-term care arrangements. MTA provides a stable, supportive environment with essential services to help individuals transition to more permanent housing.
- Level 2 and Level 3 Support Coordination
Our support coordination services are designed to assist individuals with complex needs in accessing and coordinating the various services and resources available to them. We offer Level 2 and Level 3 support coordination, where our specialists work directly with participants to create tailored plans that align with their needs, goals, and preferences. Our team helps navigate the National Disability Insurance Scheme (NDIS) and other services, ensuring that participants have access to the full range of supports they are entitled to, from healthcare services to community participation programs.
