Group-Based & Community Social Supports
At VHCC, our Group-Based and Community Social Supports are designed to provide structured, meaningful opportunities for participants to build capacity, develop social connections, and engage actively in their communities.
These supports extend beyond centre-based programs — incorporating real-world community experiences that promote independence, confidence, and inclusion, while aligning with each participant’s NDIS goals.
We deliver person-centred supports across both group and community settings, ensuring participants are supported to engage in a way that is purposeful, structured, and outcome-focused.
Our programs are designed to support both individual development and meaningful social connection.
These supports are currently offered across:
- New South Wales (NSW): Hunter Region, Newcastle, Lake Macquarie and surrounding areas
- Victoria (VIC): Box Hill, Ballarat, Shepparton and surrounding areas
What We Offer
VHCC provides a range of structured group-based and community social supports, including:
- Centre-Based & Group Community Programs (Day Programs & Outings)
Structured programs delivered across both centre-based and community environments, combining routine, engagement, and skill development with supported group outings that build confidence and independence in real-world settings. - Community Social Supports
Small-group supports delivered in real-world environments, promoting meaningful participation, inclusion, and increased independence within the community. - After-Hours Social Programs (NSW – The Social Collective)
The Social Collective is VHCC’s after-hours, community-based program delivered across New South Wales. It provides structured social engagement opportunities in real-world environments, supporting participants to build confidence, develop social skills, and increase independence outside of standard daytime supports.
What We Support
Through our Group-Based and Community Social Supports, we assist participants to:
- Build social confidence and develop meaningful relationships
- Strengthen communication and interpersonal skills
- Develop emotional regulation and appropriate social responses
- Increase independence in community participation
- Build capacity to engage in group settings and shared environments
- Explore interests, hobbies, and recreational activities
- Develop routine, structure, and engagement in daily activities
- Increase confidence in accessing community and social environments
Our Approach
Our approach is grounded in person-centred practice and aligned with the NDIS Practice Standards, with a strong focus on capacity building, active support, and participant choice and control.
We deliver supports that are:
- Structured and program-based, with clear goals and outcomes
- Focused on skill development and increasing independence
- Delivered using active support practices to encourage participation
- Strengths-based, building on each participant’s abilities
- Facilitated across both centre-based and real-world community environments
- Responsive to individual needs, including psychosocial and complex presentations
We ensure that all supports are meaningful, purposeful, and contribute to each participant’s individual goals — not just attendance.
Structured & Outcome-Focused Programs
At VHCC, all group-based and community social supports are delivered with clear intent and aligned with participant goals and funded supports under the NDIS.
Programs may include:
- Centre-based skill development programs
- Supported group outings and community activities
- Small-group community participation supports
- Social and recreational group activities
- Life skills development within both group and community settings
- Structured routines and engagement programs
All supports are designed to promote meaningful engagement, inclusion, and skill development across both structured and real-world environments.
Who We Support
VHCC provides these supports to participants with:
- Psychosocial disability
- Intellectual disability
- Physical disability
- Complex support needs
All supports are tailored to each participant’s individual goals, functional capacity, and NDIS plan.
Funding Information
Group-Based and Community Social Supports may be funded under the National Disability Insurance Scheme (NDIS) within:
- Core Supports – Assistance with Social and Community Participation
- Capacity Building Supports – Increased Social and Community Participation
The specific funding category will depend on the participant’s individual NDIS plan and goals.
VHCC works closely with participants, families, and Support Coordinators to ensure supports are delivered in line with available funding, while maintaining a strong focus on capacity building, meaningful engagement, and long-term independence.
