Chloe

Hi, my name is Chloe, and I am one of the Senior Support Coordinators at Victoria Home and Community Services.

I have been working in the NDIS sector for over five years. During the COVID-19 pandemic, I studied Mental Health after recognising the significant impact the restrictions had on people's wellbeing and the increasing need for quality mental health support. I am currently studying a Diploma of Counselling and have completed Mental Health First Aid training to further strengthen my knowledge and skills.

I support participants across all parts of Australia. Wherever possible, I enjoy providing face-to-face support, while also delivering effective and responsive Support Coordination remotely for participants in regional, rural, and interstate locations. I am passionate about empowering people to build their capacity, navigate the NDIS with confidence, and achieve their individual goals. My focus is on helping participants develop the skills, supports, and independence they need to live fulfilling and meaningful lives.

Experience & Approach

I take a strengths-based, person-centred approach to my work and pride myself on being respectful, compassionate, and non-judgemental. My own lived experience, along with supporting family members living with mental health challenges and disability, has given me a genuine understanding of the importance of empathy, advocacy, and building trusting relationships. I have extensive experience supporting people with mental health conditions, psychosocial and physical disabilities, complex family dynamics, and participants living in Supported Independent Living (SIL) environments. I am confident working collaboratively within multidisciplinary teams to ensure participants receive coordinated, holistic support that is tailored to their individual needs.
